The first few pounds are water. Yes after that your body (not the medicine) start burning fat because you are not eating as much. When you eat less your body looks for a source of energy and since there is no food to creat that energy it turns to the fat and uses that. When we over eat our bodies only need a small amount of it and therefore leaves the rest to turn to fat. So even though the meds don't burn fat the help you in other ways and make you drink water so what you loose is not water but actual fat.
